When you see a doctor outside of the dialysis unit (for example, your family doctor, a doctor at a walk-in clinic or a specialist) it is important to let the dialysis unit know about any changes that have been made to your medications.
These changes could include starting a new medication, changes in the dose of a medication you are already on or stopping a medication that you have been taking.
We need to know about medication changes so that when your dialysis doctor looks in your chart they have a complete list of your medications to use for assessing you and making changes as needed.
Please let the dialysis unit know if you decide to start filling your medications at a different pharmacy. It is important to let us know so that any new prescriptions are faxed to the right place.
If you decide to switch pharmacies and you still have refills left on your prescriptions it is your responsibility to transfer the prescriptions yourself.
The process for this is simple:
1. Call the pharmacy you have decided to move to and let them know you want to transfer your business to them.
2. Let them know which pharmacy has the prescriptions right now (name of the pharmacy, location and phone number).
3. Let them know which medications you want them to fill so they are sure to get a complete list from your old pharmacy.
They should also let you know if any of the prescriptions cannot be transferred (for example, some other pain prescriptions by law cannot be transferred between pharmacies).
