December 2017 | News, Staff News

Manitoba Renal Program has launched an updated kidney disease referral pathway in a new interactive format. Primary care providers can enter eGFR, ACR and if hematuria is present and get a course of action to follow – whether referral or primary care management. Click on ‘Primary Care Providers’ on our kidneyhealth.ca landing page to be taken directly to the tool.

Tool 1: Online Kidney Disease Referral Pathway Tool
• Enter eGFR, ACR and if Hematuria present
• Get a course of action to follow – whether referral or primary care management

Tool 2: Kidney Failure Risk Equation Tool
• Identifies risk of kidney failure within three and years and current stage of kidney disease (if present).
You can find an explanation of the pathway and risk equations here. Below is a ‘tour’ of the new tool and primary care section of our website.
