Manitoba Renal Program has launched an updated kidney disease referral pathway in a new interactive format. Primary care providers can enter eGFR, ACR and if hematuria is present and get a course of action to follow – whether referral or primary care management. Click on ‘Primary Care Providers’ on our kidneyhealth.ca landing page to be taken directly to the tool.


Tool 1: Online Kidney Disease Referral Pathway Tool
• Enter eGFR, ACR and if Hematuria present
• Get a course of action to follow – whether referral or primary care management


Tool 2: Kidney Failure Risk Equation Tool
• Identifies risk of kidney failure within three and years and current stage of kidney disease (if present).
